    The Commonwealth of Massachusetts has an urgent need for Personal Protective Equipment (PPE) for healthcare workers. Hospital and health networks fighting on the front-lines of the Commonwealth’s COVID-19 response face immediate shortages of the equipment necessary to keep them safe from contagion while caring for our community.

    As part of a coordinated response between academic, industry, and government, we are indexing the Commonwealth’s manufacturing capacity to produce critically-needed components.

    If your organization is NOT located within the State of MA, please still complete this form. We will use this for coordinating efforts between other state and federal initiatives.

    Instructions for completing this form:

    — Although we are based in Massachusetts, please complete the form even if your production facilities are located elsewhere. This information will be critical for coordinating parallel efforts across the nation.

    — Only one response per production facility should be submitted. Please coordinate a central contact for organizing your organization’s response per production facility.

    — Organizational contacts should be available to coordinate the deployment of production assets. If the person completing the form is not this person, there is space provided for additional contact information.

    — Data from this form is shared only with the Massachusetts Manufacturing ERT.
